As per the current Guidelines from the HPSC (HPSC Childcare Guidance (V1.2 31.07.2020)), it is not a requirement for a medical certificate from a medical practitioner to ensure attendance at or return to school/childcare after an absence.
Instead a ‘Parental Declaration Form’ (which can also be found in Appendix 1 of the attached link above) can be signed to confirm parental belief that the child has sufficiently recovered to return to school, that they have adhered to any medical advice given regarding school/childcare exclusion periods and that to the best of their knowledge, the child represents no infection risk to other children or staff.
