Chronic Disease Management Programme

The HSE’s Chronic Disease Management programme provides structured monitoring of people with long term chronic illness. Its aims are to improve your health and wellbeing and give you confidence in managing your own condition.

It is a free service and available for those aged 18 years and older who have a medical card, GP Visit Card or a Health Amendment Act card and have a specified chronic disease such as 

  • COPD
  • Asthma
  • Type 2 Diabetes
  • Hypertension
  • Cardiovascular disease including heart failure, heart attack, angina, stroke, irregular heart beat (atrial fibrillation), 

The programme offers twice yearly, structured reviews where you will see both a nurse and doctor who will help you to better understand and manage your condition. The emphasis at these appointments will be on lifestyle & medical advice,  disease management, risk factor control & the creation of a patient care plan between you and the GP.

If you have received a text message from us with regard to the Chronic Disease Management Programme or you have one of the above conditions, you can contact reception to make an appointment and start the registration process. You will then be given an appointment with the nurse / GP.
